Get To Know You
Having different formats for an “All About You” form is important because students process information and express themselves in different ways. Offering multiple formats increases accessibility, reduces anxiety or writing fatigue, and helps students feel more comfortable sharing, which leads to more accurate information and stronger early relationships with YES Navigators.
Permission Slip
During the initial meeting with student, handout the Permission Form for student to take home. Once student brings back signed Permission Form, upload it into the Interactions Tracker.

Call Home Script
This call takes place after the initial student check-in and before ongoing support begins so parents are fully informed about the program, understand the importance of Permission Form sent home, and have the opportunity to give consent. Early communication also helps build trust and ensures families feel included as partners in supporting their student.
